Telemedicine Takes Transgender Care Beyond The City

*Medical research has done wonders to rid populations of diseases; however, the U.S. health care system has failed to appropriate the right resources to Medicare patients with one or more chronic conditions. Health Quality Partners in Doylstown, PA enrolls Medicare patients with at least one chronic illness and hospitalization and sends a trained nurse to see them on a routine basis, whether they are healthy or sick. As a result, the HQP program has reduced hospitalizations and cut Medicare costs.

Across the country, hospitals are embracing intensive intervention programs to help victims of violence — including those who have criminal histories — after they have been brought in for treatment of injuries. Such programs can help prevent retaliation, reduce the chance a patient will be violently injured again, and put people on track for success.

Harm reduction is a quickly growing practices that aims to reduce drug-related deaths and the spread of disease often caused by dirty needles. Although public perception is still widely negative, the industry has seen success by using the act of offering clean needles as a first point of contact to offer other health and medical services.

LGBTQ asylum seekers face a unique set of problems coming to the United States, often from places where gender expression and sexuality are strictly regulated. Four organizations across the US, Mexico, and Canada are filling this unique niche. For example, AsylumConnect created an app of resources on how to apply for asylum and a catalogue of LGBTQ-friendly services and organizations in the US for them to learn about. All four aim to validate LGBTQ asylum seekers and keep them safe.

DOROT is a non-profit organization that matches seniors with younger adult volunteers for social visits to combat isolation and loneliness and to promote intergenerational friendships. Many of the seniors are widowers and social isolation can lead to negative health consequences. The organization also delivers fresh meals and holds other events, but was unable to operate as usual once the Covid-19 pandemic hit, so DOROT helped seniors set up Zoom for online conferencing. The group has matched 500 seniors with volunteers and advises similar programs across the country. Many pairs have formed deep friendships.

The Chicago Black Drag Council launched after a series of protests that called on Chicago’s LGBTQ neighborhood to address racial discrimination and oppression. A handful of businesses quickly agreed to a live-streamed town hall to discuss ways to increase inclusivity. As a result, a prominent host of popular drag shows was ousted for racial discrimination and the business chamber dropped the neighborhood’s nickname, “Boystown,” from marketing materials because it is not inclusive. The Drag Council has also raised tens of thousands of dollars in cash and supplies to support Black- and trans-led initiatives.

Fearing that consistent homophobic street harassment by a resource center for queer youth was detrimental to their mental health and safety, activists set up a weekly counter protest to drown out the harmful messages. The counter-protest grew from a single person to the Pride Corner on Whyte movement with several hundred social media followers. Over time, the participation of dozens of unhoused and housed queer and trans youth provided opportunities for social service outreach, including a mentorship program that paired youth with older queer and trans participants who offer friendship and emotional support.

Several community organizations and municipalities like the Office of Transgender Initiatives (OTI) have come together to decrease homelessness rates among the transgender population. OTI formed a Trans Advisory Committee which focused on budget and policy advocacy based on input from the transgender community to address homelessness and partnered with efforts like the Transgender District. All in all, these combined efforts have decreased transgender homelessness by 15% since 2019.

Birth in Color's doulas are filling the maternity care gap as rural labor and delivery rooms close, disproportionately impacting people of color. Doulas provide physical, emotional and informational support before, during and after childbirth, and advocate on the mother’s behalf. Doula care leads to improved birth outcomes and lower rates of C-sections, and in April 2022 doula services were added under the state’s Medicaid program.

Health engagement hubs like Washington state’s Buprenorphine Pathways connect people with prescriptions for methadone and buprenorphine to help treat addiction. In addition to medication and a syringe-exchange program, the hub also connects patients to social services, taking a more holistic approach to addiction treatment.
